The number of actual organ donors in Selangor has seen a concerning decline, falling from 23 donors in 2023 to 18 in 2025. This data was shared by Selangor Public Health and Environment Committee chairman Jamaliah Jamaluddin during a state legislative assembly sitting on Tuesday, April 28, 2026. In tandem with the lower headcount, the state’s donation rate per capita has also dropped from 3.19 per million population in 2023 to 2.43 per million in 2025.

Despite the decrease in physical donations, Selangor continues to lead the nation in terms of intent, ranking fourth overall for organ donation pledges with a rate of 1.43% of the population. Between 2023 and 2025, the state recorded 16,242 new pledges, which remarkably represents 27% of the total national pledges (more than 58,000) for that same period. Jamaliah noted that while the state's actual donation rate is currently down, it remains higher than the national average.

To address the gap between pledges and actual donations, the state government is ramping up its "Satu Ikrar, Lapan Harapan" awareness campaign in collaboration with the National Kidney Foundation of Malaysia. These efforts focus on community education and addressing cultural or religious misconceptions, specifically clarifying that organ donation is permissible in Islam to reduce social stigma. Nationally, the National Transplant Centre (NTC) is working to streamline the donor pledge process via MySejahtera, as over 10,000 Malaysians remain on waiting lists.
